背景与目的:超声是体检发现胰腺实性假乳头状瘤(solid pseudopapillary tumor of the pancreas,SPTP)的重要手段,本研究对SPTP的超声表现及其诊断价值进行探讨。方法:收集复旦大学附属肿瘤医院经手术病理证实的62例SPTP的超声检查资料,并进行总结分析。结果:本组中5例患者超声检查漏诊。SPTP的超声声像学特征包括:肿瘤多较大,以胰体尾多见,边界可较为清楚,形态较为规则,多呈低回声,或者为低回声伴无回声,内部回声多不均匀,少数患者可有钙化,病灶内多无明显血流,多无主胰管及胆总管扩张,多无区域淋巴结肿大。结论:SPTP的超声表现有一定特征性,具有重要的临床应用价值。
Abstract
Background and purpose: Ultrasound is a regular screening method of solid pseudopapillary tumor of the pancreas (SPTP). This study was to summarize the diagnostic value of ultrasound to SPTP. Methods: Clinical and ultrasound data of 62 SPTP cases in Fudan University Shanghai Cancer Center were retrospectively collected and analyzed. Results: Five cases of SPTP were undetected by ultrasound in the group. The features of ultrasound including: large mass located at the body and tail of the pancreas
clear boundary and regular shape
low ultrasound with uneven signal
or low signal mixed with no signal. A few cases have calcification and blood signal. Most of the cases presented no dilation of main pancreatic duct and bile duct and regional lymph nodes enlargement. Conclusion: Ultrasound can be used to detect SPTP which has special ultrasound signal features.
